A free 15-minute telephone consultation is available for questions to be asked, assess if treatment needs can be met, and to ensure that a client feels comfortable with their therapist, which is imperative for treatment success before an appointment is scheduled. This is not a counseling/therapy session. Referral assistance offered if needed.
Payment for services are due at time session paid at the beginning of appointment. This office does not balance bill for services rendered.
Payments for psychotherapy services are made by cash, or check.

A fee will be charged for all returned checks.
If unable to keep your scheduled appointment, 48-hour notice is required so the needs of all clients of Kontos Psychotherapy LCSW PC can be met.
If 48-hour notice is not given, Kontos Psychotherapy LCSW PC reserves the right to charge a fee for the missed session as per the signed treatment agreement.
Insurance companies do not pay for missed or canceled appointments
The office has voice-mail and a message can be left. Messages are checked routinely.
Please do not use email and/or text messaging for any form of communication as they will not be responded to. They are not secure means of communication as per HIPAA privacy laws. Telephone communication is the only accepted means of communication.
Sliding scale fee is available for clients who have a demonstrated financial need. Please discuss your financial needs with your therapist.

If your plan is not accepted, Kontos Psychotherapy LCSW PC is also an ‘Out of Network’ Provider. To determine if you have ‘Out of Network’ coverage, check with your insurance provider and ask the following questions:
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- What is percentage of reimbursement?
- Is approval required from my PCP (primary care physician)?
Payment of service is expected at time of session. Completed claim forms will be provided directly to you for submission to your insurance carrier for reimbursement directly to you. Contact your insurance carrier directly for information regarding rules, and regulations regarding reimbursement.
For claim form to be submitted, a mental health diagnosis for psychotherapy services must be given. Not all diagnosis guarantee reimbursement. For this reason may clients elect to pay in cash to ensure their full confidentiality. For more information regarding this, please ask.
The law protects the relationship between a client and therapist and information cannot be disclosed without a signed Authorization of Release form. As a NYS mandated reporter There are a few exceptions to this law:
- Suspected child abuse or dependent adult or elder abuse
- A serious threat of bodily harm to another person/s
- An intent to harm oneself.
In these instances, information will be disclosed without written permission to the appropriate authorities immediately to ensure safety for all parties involved.
