DWI/DUI Impaired Driver

 Evaluations 

A person charged with operating a motor vehicle under the influence of alcohol or drugs, who refused to submit to a chemical test may be required to complete a mandatory evaluation. This evaluation must be completed by an approved provider by the Office of Alcoholism and Substance Abuse Services (OASAS) who is a licensed professional.  

Kontos Psychotherapy is an approved provider!

According to New York State Guidelines, the evaluation process will take approximately two hours, held in one -  two hour session or two - one hour sessions as determined by evaluator.  The evaluation process includes obtaining a personal history, a health history, an alcohol and subtance abuse history, a collateral interview, contact with referral sources, urine and breath test, and a written report. 

Upon completion of the evaluation, a report will be written, discussed with client and submittd which will include evaluations results and treatment recommendations/referals if needed.

An Abstract of Driving Record (view sample here) is required for evaluation to be completed. This can be obtained at Department of Motor Vehicle. For more information call Dept. of Motor Vehicles at 518-486-9786 Monday-Friday 8am-4pm or go to your local office.
